Overview
Bai Hu Jia Gui Zhi Tang — “White Tiger plus Cinnamon Twig Decoction” — is from Zhang Zhongjing’s Jin Gui Yao Lue. It adds Gui Zhi to Bai Hu Tang to address the classical “Wen Nue” (warm malaria) pattern: high fever with chills, joint pain, no sweat and trapped Heat. In modern practice it is widely used for acute hot Bi syndrome — inflammatory joint conditions with high fever, severe joint pain and the burning, red, swollen presentation of acute rheumatic fever, acute gout flare and acute rheumatoid flare.

TCM pattern
Bai Hu Jia Gui Zhi Tang is prescribed for intense Heat with Wind-Damp obstruction:
- High fever, severe thirst
- Severe joint pain with red, hot, swollen joints
- Possible chills alongside fever
- Marked restlessness, irritability
- No sweat or sweat that doesn’t relieve the heat
- Tongue — red, dry yellow coat
- Pulse — surging, slippery, rapid

- Shi Gao (Gypsum Fibrosum, 30–100g) — chief; clears intense Qi-level Heat
- Zhi Mu (Rx. Anemarrhenae, 6–15g) — assists Shi Gao; nourishes Yin
- Geng Mi (rice, 9–15g) — protects Stomach Qi
- Zhi Gan Cao (Rx. Glycyrrhizae Preparata, 3–6g) — harmonises
- Gui Zhi (Ram. Cinnamomi, 6–9g) — warms and unblocks the channels; reaches the joints

Cautions
Strictly for the Heat-Bi pattern — not for cold or deficient Bi syndromes.
