San Wu Bei Ji Wan (三物备急丸) — Three Substance Emergency Pill

San Wu Bei Ji Wan is a heroic and dangerous classical Chinese formula from the Jin Gui Yao Lue for acute severe Cold accumulation in the abdomen with intestinal-obstruction presentation — sudden severe lower abdominal pain, severe constipation[11], cold extremities and profuse cold sweat. The three herbs — the toxic warm purgative Ba Dou (croton seed), Da Huang and Gan Jiang — violently purge cold accumulation downward. This formula is not used in modern outpatient practice — acute severe abdominal pain with obstruction is a surgical and medical emergency. Presented here for educational reference only.

Overview

San Wu Bei Ji Wan — the “Three Substance Emergency Pill” — is from Zhang Zhongjing’s Jin Gui Yao Lue. It is one of the most heroic and dangerous formulas in the materia medica, reserved for acute severe Cold accumulation in the gut with intestinal obstruction-like presentation. The three herbs — Ba Dou (croton seed, highly toxic warm purgative), Da Huang and Gan Jiang — combine to violently drive out cold accumulation downward.

In modern outpatient practice this formula is almost never used — acute severe abdominal pain with obstruction is managed surgically and medically. The formula is included here for completeness and historical interest. It belongs to the “reserve” classical formulas that experienced practitioners might use only in very specific circumstances.

I do not generally prescribe this formula in outpatient practice. It is presented here for educational reference.

TCM pattern

San Wu Bei Ji Wan is prescribed for acute severe Cold accumulation in the abdomen:

  • Sudden severe lower abdominal pain
  • Abdomen hard and resistant to pressure
  • Severe constipation — no bowel movement, no gas
  • Cold extremities
  • Profuse cold sweating
  • Strong, forceful body (not appropriate for weak or deficient patients)
  • Tongue — pale, white slippery coat
  • Pulse — deep, wiry, forceful

Key herbs

  1. Ba Dou (croton seed, Croton tiglium, 30g) — chief; powerful warm purgative; toxic; use only prepared
  2. Da Huang (Rx. et Rz. Rhei, 30g) — purges accumulation
  3. Gan Jiang (dried ginger, 30g) — warms the middle

Formula actions

  1. Warms and purges Cold accumulation
  2. Opens severe intestinal obstruction from Cold

Conditions treated

  1. Acute severe Cold abdominal pain with obstruction — historical use; modern practice uses surgery
  2. Acute food stagnation with sudden severe pain in strong constitutions — rarely

Cautions

Ba Dou is highly toxic. This formula is dangerous and is not used in modern outpatient practice. It is reserved for very experienced classical practitioners in specific historical settings. Modern Chinese hospital practice uses surgery and conventional medical care for acute intestinal obstruction.

Strictly contraindicated in pregnancy, in the elderly, in children, in weak constitutions, in any Heat pattern, in any Yin-deficient pattern.

Acute severe abdominal pain is a medical emergency. Call 999 or attend A&E.

Frequently asked questions about San Wu Bei Ji Wan

Is San Wu Bei Ji Wan still used today?

No — the formula is not used in modern outpatient practice. Acute severe abdominal pain with intestinal obstruction is managed surgically and medically. San Wu Bei Ji Wan is presented here for educational reference and historical interest only. The Ba Dou content makes it inappropriate for general clinical use.

Why is San Wu Bei Ji Wan considered dangerous?

The chief herb, Ba Dou (croton seed, Croton tiglium), is one of the most toxic purgatives in the materia medica. Even in correctly prepared form it can cause violent purging, dehydration, electrolyte disturbance and significant cardiovascular stress. Combined with Da Huang it produces an extremely heroic intervention reserved historically for life-or-death cold-pattern intestinal obstruction.

What is the TCM pattern San Wu Bei Ji Wan addresses?

Acute severe Cold accumulation in the abdomen with obstruction-like presentation: sudden severe lower abdominal pain, abdomen hard and resistant to pressure, severe constipation with no gas, cold extremities, profuse cold sweat, deep wiry forceful pulse and pale tongue with white slippery coat. Only appropriate in strong, robust constitutions — never in weak or deficient patients.

What should I do if I have severe abdominal pain?

Acute severe abdominal pain is a medical emergency. Call 999 or attend A&E immediately. Possible causes include bowel obstruction, perforation, appendicitis, ischaemic bowel and other surgical emergencies. Do not attempt to self-treat with any herbal preparation.

Who should not take San Wu Bei Ji Wan?

The formula is strictly contraindicated in pregnancy, the elderly, children, weak constitutions, any Heat pattern and any Yin-deficient pattern. In practice, it should not be taken by patients at all in modern UK or international settings — this page exists solely as a historical and educational reference for students of classical Chinese herbal medicine.
