What is Tong Xie Yao Fang?

Tong Xie Yao Fang — Painful Diarrhoea Essential Formula — is the classical Chinese herbal formula specifically designed for abdominal pain that precedes diarrhoea from Liver Qi overacting on the Spleen — the most common underlying pattern in stress-related irritable bowel syndrome. A focused, four-herb formula that simultaneously spreads Liver Qi, tonifies Spleen Qi, astringes the intestines and stops pain, addressing all four aspects of the pathological mechanism.

Tong Xie Yao Fang TCM pattern

Prescribed for Liver overacting on the Spleen (Gan Pi Bu He), characterised by: abdominal pain that precedes and is immediately relieved by diarrhoea, the pain and urgency recurring episodically (particularly after emotional stress), urgent loose stools, possible mucus in the stool, a wiry left and weak right pulse, and symptoms that clearly worsen with emotional tension or anxiety.

  1. Bai Zhu (Atractylodes macrocephala rhizome, 3-12g) — the principal herb; tonifies Spleen Qi, dries Dampness and strengthens the Spleen’s ability to resist Liver overaction
  2. Bai Shao (Paeonia lactiflora root, 2-30g) — nourishes Liver Blood, softens the Liver and alleviates cramping pain
  3. Chen Pi (aged Citrus reticulata peel, 1.5-10g) — regulates Qi, dries Dampness and relieves abdominal distension and pain
  4. Fang Feng (Saposhnikovia divaricata root, 2-10g) — expels Wind from the Spleen and Liver channels; raises Spleen Qi and dries Dampness

TCM patterns and conditions where it is used

The formula is used for the TCM pattern of Liver invading the Spleen (Liver Qi stagnation with Spleen Qi deficiency). The presentations within that pattern include:

  1. IBS with predominant diarrhoea (IBS-D) — randomised controlled trial evidence supports the formula in IBS-D[1] and systematic review and meta-analysis of 11 RCTs (n=985) reported significant improvements in stool consistency, frequency and abdominal pain[2]
  2. IBS with mixed bowel habits (IBS-M) — in TCM, often the appropriate base formula for the alternating-pattern presentation
  3. Stress-triggered digestive complaints with cramping, urgency and loose stools — the Liver-invading-Spleen pattern
  4. Liver-Spleen disharmony presentations in inflammatory bowel disease remission phase (alongside, not instead of, conventional care)

Research evidence

Tong Xie Yao Fang is one of the better-studied classical Chinese herbal formulas in contemporary clinical research:

  • IBS-D randomised controlled trial — a double-blind, placebo-controlled RCT (n=160) published in Alimentary Pharmacology & Therapeutics reported that Tong-Xie-Yao-Fang granules were associated with reductions in IBS symptom severity scores, bowel urgency and abdominal pain compared with placebo[1]
  • IBS-D systematic review and meta-analysis — review of 11 RCTs (n=985 adults) reported that TXYF was associated with global symptom improvement (moderate evidence by GRADE) and was statistically more effective than placebo for stool consistency, stool frequency and abdominal pain[2]
